BARIATRIC

SURGERY REVIEW

Countless medical studies and research projects have shown that bariatric surgery is the most effective means for sustained weight loss. However, these procedures are not a panacea.

Some patients lose less than the expected and desired amount or regain the lost weight. In these cases, revision surgery may be necessary.

At Minimally Invasive and Bariatric Surgery (MIB), Dr. Abel Bello thoroughly analyzes all factors of the previous surgery that may have resulted in its failure. Almost all bariatric procedures can be revised or readjusted to maximize results and patient satisfaction.

What is revisional bariatric surgery?

"Revisional bariatric surgery" is a general term that refers to any number of bariatric surgical procedures that a patient may undergo to correct a previous surgery. The need to revisit a surgery is not uncommon. Approximately 10-20% of all bariatric patients will require a revision procedure due to weight gain or complications from surgery. Complications include:

  • Vomiting
  • Reflux
  • Stenosis or blockages
  • Fistula
  • Marginal ulcers
  • Excessive weight loss / malnutrition
  • Device malfunction, including band erosion, band prolapse and pouch dilatation, are less common, but may still be the reason a patient needs revision surgery.
  • Complications caused by device malfunction would also require further medical attention.
  • For all patients seeking revision, an individual in-person consultation is required. Subsequent follow-ups may also be required to ensure that the correct steps are being taken regarding diet and exercise plans.

Why does bariatric surgery sometimes fail?

There are several reasons why a patient's bariatric procedure may "fail". Sometimes, the devices are defective; they move or stop working. Other times, the patient is unable to follow the program's diet and exercise regimen. Bariatric procedures are a stepping stone to a healthier lifestyle, not a panacea. Once you have undergone the procedure, your efforts are just beginning. Otherwise, you may find yourself following unhealthy habits and will need more surgeries to maintain your health.

Can I have my procedure at MIB if I had my first procedure elsewhere?

Yes, absolutely! Even if you had your previous surgery at another facility, Dr. Bello can perform your bariatric review and make sure everything goes well this time.

What options are available for revision surgery?

Certain bariatric procedures cannot be reversed, but most can be revised or corrected if problems arise. For example, gastric bypass is irreversible because it requires removal of part of the gastrointestinal tract. Other procedures, such as lap banding, can be replaced, readjusted or removed.

Are there major risks of revision surgeries?

Revision procedures are not inherently more dangerous than a normal bariatric procedure; however, consideration should be given to any scarring from the original procedure that may cause additional difficulties.

Who qualifies for revision bariatric surgery?

If you qualified for bariatric surgery, you are likely qualified for a revision bariatric procedure. Dr. Abel Bello will evaluate you on a case-by-case basis, determine what went wrong with the first procedure and create a customized plan to eliminate the problems discovered. In this way, you will pave the way to lasting weight loss and improved health.
